An Advance Care Plan is a legal document that allows you to give instructions in advance regarding the medical treatment you wish to receive in the event you lack the capacity to in the future.
An Advance Care Plan contains your instructions regarding the medical care you wish to receive in the event you are not able to communicate those instructions yourself, and may include the continuation or termination of life support. With an Advance Care Plan, you can also appoint a health care agent and make an anatomical gift.
Without an Advance Care Plan, the decisions regarding your medical treatment could be left for someone else to make and your actual wishes may be confusing or simply unknown to that person. When you execute an Advance Care Plan, you make your treatment wishes known to your family and health care providers and avoid the awkward and potentially painful “guessing game” of what you would truly want.
